Aug. 19, 2007 TO: Ohio Northern University Student-Athletes and Their Parents FROM: Mr. Tom Simmons, Director of Athletics & Mr. Kurt Wilson, Head Athletic Trainer SUBJECT: ONU Supplemental Accidental Medical Insurance Coverage for Athletic Injuries I realize medical insurance coverage and claims processing can be a complicated and confusing process, especially with children in college living several miles from home. Athletics sets additional hurdles with the need for prompt and efficient medical care that often cannot occur due to limited insurance coverage. While your insurance may provide adequate medical coverage for your son or daughter at home, it may be limited with providing local coverage in the Ada/Lima area, especially if you belong to an HMO, and certain PPO’s. To assist you with making the most informed decisions regarding medical insurance coverage for your son and/or daughter while they are participating as an ONU intercollegiate athlete, I encourage you to read the following information concerning the | Ohio Northern University Student Medical Insurance | and | Ohio Northern University Supplemental Accidental Medical Insurance Coverage for Athletic Injuries |. Please note that these are two separate plans of coverage and are outlined in the attached letter. If purchased, the Ohio Northern University Student Medical Insurance becomes your son/daughter’s PRIMARY insurance, and other medical insurance coverage should be reported as secondary insurance. This MUST be noted as such when filling out medical paperwork. This is full 12-month coverage, and is NOT limited to athletic injury or illness! You should have received information regarding the | Ohio Northern University Student Medical Insurance | plan throughout the summer from the ONU Controller’s Office. If you did not, please contact the ONU Controller’s Office. Please note that your son or daughter will be automatically enrolled in the ONU Student Medical Insurance Plan unless you return the required paperwork declining coverage. The second plan is the | Ohio Northern University Supplemental Accidental Medical Insurance Coverage for Athletic Injuries |. This plan is well-detailed in the attached letter, and is automatically provided for ONU intercollegiate athletes sustaining athletic-related injuries in a supervised capacity. I strongly encourage you to call me with any questions regarding the Ohio Northern University Supplemental Accidental Medical Insurance Coverage and/or how the | Ohio Northern University Student Medical Insurance | can assist with the coverage of intercollegiate athletic injuries. My goal at ONU is to minimize as much out-of-pocket expense to the athletes and parents as possible when athletic injuries occur.
